Be sure to include current part pricing shipping costs with your purchase order generation. You can do this by processing an order on this site's online store (http://www.andymark.biz) before adding in a charge card.
To avoid processing delays, please include AndyMark item numbers (am-xxxx) and the required shipping cost for your order. These can be found on our web site; just build your “shopping cart” and apply shipping Zip Code/State. You do not need to enter charge card information. Here is a link to a
blank PO worksheet (.pdf file) for assisting customers in creating an order. This form may help create the PO, but we will still need a faxed or emailed copy of your organization's official PO paperwork.
For Indiana customers, if you are tax exempt as it relates to the purchase of our product, you must include your State Tax ID Number.
